The CHS Adolescent Outpatient Treatment Program provides integrative therapeutic services for adolescents in need of outpatient substance abuse treatment. The Program offers comprehensive, confidential counseling services for individuals by incorporating evidence based treatment approaches which include individual and group counseling, family counseling, psycho-education, life skills, experiential therapy, relapse prevention, and formal referrals to community agencies based on individual needs.

These outpatient services are offered in a traditional agency setting or are provided in the home of the individual and family receiving the services and can be delivered in even more nontraditional settings located in the community but are convenient for the individual /family participating in the program.

Screenings are used for the early identification of individuals at potentially high risk for a specific condition or disorder. Most screenings are used for the basis for further evaluation, preliminary intervention or a possible referral to a specific program or service. They are generally brief and narrow in scope and can be administered as part of a routine clinical visit.

Assessments provide a more complete clinical picture of an individual, is comprehensive in focusing on the individual’s functioning across multiple domains, can aid diagnosis and/or treatment planning in a culturally competent manner and can identify psychological problems and conditions, indicate their severity, and provide treatment recommendations.

CHS uses a number of screenings and assessments that are reliable, can be validated and will direct the patient or client to the most appropriate treatment program or behavioral health service that meets their needs and/or the needs of their families. The following is a list of the screenings and assessment instruments used by CHS professionals:

-KIS – Knight Software Screening and Assessment
-ASI – Addiction Severity Index and individual/family narratives
-ADAD – Adolescent Drug Abuse Diagnosis
-ASAM – American Society of Addiction Medicine criteria used to determine level of care needed
-Mental Status Exam and Psychiatric Evaluations when needed
-Medical Exam to determine the medical necessity for treatment and other services

All treatment, prevention and intervention modalities provided to patients, clients and their families are Evidenced Based, Culturally Competent and Patient Centered. The modalities used include:

-Individual and Group Therapy
-Cognitive Behavioral Therapy
-Trauma Informed Treatment Modalities
-Multi-dimensional Family Therapy
-In-Home and On-Site Therapeutic Counseling and Support Services delivery and referral
-WRAP - Wellness Recovery Action Plan

-Medical, Psychiatric and Bio-Psychosocial Assessments
